< 1.5 cm, age < 65Imaging yearly × 5, then every 2 yr × 2; stop if stable at 9 yr
< 1.5 cm, age 65–79Imaging every 2 yr × 5; stop if stable at 10 yr

> 2.5 cm, low riskEvery 6 mo × 4, then yearly × 2, every 2 yr × 3
> 2.5 cm, high riskEUS-FNA and surgical consultation
Serous cystadenomaSurgical consult if > 4 cm
Age ≥ 80 at detectionEvery 2 yr × 2 if a surgical candidate; stop if stable

  • Significant growth: ≥ 100% for cysts < 5 mm, ≥ 50% for 5–14 mm, ≥ 20% for ≥ 15 mm (long axis).
  • Follow-up stops at age 80. Tiny “white dot” cysts: one stable study at 2 years is enough.
  • Imaging: contrast-enhanced MRI/MRCP or pancreas-protocol CT.
